Nurse: I’m afraid not. He won't be free until 11:30.

Notes:

arrange for 是习惯用语，意思是：安排，准备

I'm afraid not 较为客气的口语表达方式.用来委婉的否定。纠正和驳斥对方。

如：

Can you finish the work tomorrow?你明天能完成这项工作吗？

I'm afraid not.(我想）恐怕不能。

not ...until ... 是连用结构。意思是‘到...时， 才.....’
